How to claim each tube

At checkout, paste one of your sample codes (from your approval email) in the "Discount code" field.
Total goes to $0.00. Fill in your shipping info (ship to your practice address) and place the order.
Repeat with each of your 5 codes if you want all 5 tubes. Each code is single-use.

You can request another batch of 5 codes after 30 days. Submit a refill request with the same email and we'll auto-approve within 24 hours.
